What the work actually is

You are the clinical check on a model. The tasks are usually one of three things:

  • Reviewing clinical content. A micro1 hospitalist listing asks you to review inpatient documentation, including the history, assessment and plan. A family medicine listing asks you to annotate and review office notes, referrals and lab or imaging results for accuracy, clinical soundness and completeness.
  • Grading model answers. You read two or more AI responses to a medical question, score them against a rubric and write a short reason a second reviewer can check. "Response A misses the drug interaction" is the kind of note projects want.
  • Writing model answers. You write the hard question or the ideal answer yourself, so the model has something correct to learn from.

There is no patient contact and no prescribing. For the general picture of how these tasks work, see what an AI trainer does.

What it pays: ranges listed on roles we track

On 26 October 2026, 54 open roles match this guide and 41 of them list an hourly rate in US dollars. The ranges listed on roles we track run from $15 to $250 an hour. Taking the midpoint of each range, half of the roles sit between $100 and $170 an hour, with a median of $102.50.

These are the ranges written in the postings, not what anyone earned. A wide range usually means pay depends on your specialty, your experience or the project. Some postings list only a top rate, and some list no hourly rate at all.

What they screen for

  • Degree and licence. The listings ask for an MD or DO. Several US roles add board certification (internal medicine, pediatrics, hospital medicine or family medicine) and an active licence. One Mindrift listing asks for an unrestricted US licence verified with the medical board and says it is not a fit for students, unlicensed graduates, nurse practitioners, PAs, nurses or pharmacists.
  • Recent practice. The micro1 hospitalist listing prefers inpatient experience within the last two years.
  • Location. Some roles are for people based and licensed in the United States. Others are open to many countries, and a few ask for a second language.
  • An AI interview. micro1 runs a recorded interview with an AI interviewer, usually 20 to 40 minutes, tailored to the role. Read the micro1 AI interview guide before you start it.
  • ID verification. After the interview you verify your identity. See what happens after the interview.

How long it takes to get started

The application steps are quick: the interview is under an hour, and ID checks clear in minutes or, for some documents, up to 72 hours. The wait that matters comes after that, because you only get paid work when a project picks you.

No platform publishes a typical wait for physicians. Candidate reports are mixed: some start within days, others wait weeks, and some are never matched. We cannot tell you which one you will be. It depends on which projects are open for your specialty that month.

The honest downsides

  • It is project work and it can end. micro1 says roles continue as long as the client has work. Mindrift describes its opportunities as project-based.
  • Hours vary. A Mindrift listing estimates 10 to 20 hours a week during active phases. Between phases there can be nothing. Some roles pay per task, not per hour.
  • You are a contractor. No benefits, no paid leave, and you handle your own taxes. See micro1 payments and taxes.
  • It does not replace clinical income. Plan on it as extra work unless you have seen a steady stream of it for months.
  • Check your employer. Some hospitals and groups have rules on outside work and on working with AI companies. That is for you to confirm.
